Complex Oncology Nurse Navigator

This role is for a Complex Oncology Nurse Navigator at Thyme Care, a company that uses technology and AI to improve cancer care. The navigator will provide high-touch human support to members diagnosed with cancer, coordinating care, monitoring health, and educating members and caregivers. What you'd actually do

  1. Conduct clinical assessments, monitor for changes in health, coordinate care, including transitions, and educate members and caregivers about their diagnosis and treatment over the phone to support our higher-acuity members as they move through the oncology care continuum.
  2. Identify and prioritize a member's needs and help them remain safe in the community.
  3. Assist members with care coordination and care management following admissions.
  4. Coordinate discharge plans with hospital case managers and follow-up care with providers.
  5. Monitor member progress, provide regular updates, and establish targeted support plans with the healthcare team in case conferences.

Skills

Required

  • Active, unrestricted compact Registered Nurse (RN) license
  • Degree in Nursing (ADN or BSN)
  • At least 5 years of nursing experience
  • 3 years of high-acuity, adult oncology experience
  • Certified as an Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM)
  • Strong clinical focus
  • Culturally competent care
  • Effective communication and listening skills
  • Ability to juggle multiple tasks and work under pressure
  • Comfort with ambiguity
  • Experience with video chatting, Google Suite, Slack, electronic health records, or comfort in learning new technology
