Nurse Manager, Population Health

Thyme Care is a healthcare company focused on improving the experience for people with cancer. They combine human support with technology and AI to coordinate care. This role is for a Nurse Manager, Population Health, who will lead nursing teams, coach Nurse Team Leads, and ensure high standards of clinical care and efficiency. The role involves people management, performance monitoring, and ensuring compliance with NCQA workflows.

What you'd actually do

  1. Directly manage Nurse Team Leads, providing coaching, mentorship, and structured performance management strategies to help them become strong and effective leaders.
  2. Enable Nurse Team Leads to improve team performance, productivity, and efficiency through people management practices, including clear goal-setting, regular feedback, accountability systems, and engagement strategies.
  3. Set and monitor performance metrics across multiple teams, using data to identify trends, address barriers, and implement targeted improvement plans.
  4. Serve as an escalation point for complex member and caregiver cases, modeling sound clinical judgment while supporting team confidence.
  5. Build strong communication loops across leadership and frontline nurses, ensuring insights from staff and members shape organizational decisions.

Skills

Required

  • Bachelor of Science Degree in Nursing
  • unrestricted Registered Nurse (RN) license
  • Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM)
  • 8 years of nursing experience
  • 2 years as a nurse leader in a remote oncology navigation and/or remote oncology case management environment
  • proven leadership experience with a strong track record of coaching, mentorship, and holding teams accountable to drive results
  • experience designing and implementing structured performance management strategies
  • experience applying data-driven approaches using performance metrics and outcomes
  • experience aligning leadership development and team performance strategies with organizational goals
  • experience leading teams through organizational change and growth

Nice to have

  • willingness to obtain additional state licenses, as needed
  • Prior startup experience is preferred

What the JD emphasized
