Thyme Care is seeking an Oncology Nurse Navigator to provide clinical support, triage, and education to members with cancer. This role involves conducting clinical assessments, coordinating care, managing symptom management, and facilitating communication between members, their care network, and providers. The role utilizes technology and AI to enhance care delivery but does not involve building or directly working with AI models.

What you'd actually do

  1. Provide triage, support, and education to members throughout their cancer journey via phone and email.
  2. Conduct comprehensive clinical assessments, oversee member health, and facilitate end-of-life care discussions.
  3. Actively engage with members, address clinical issues, and efficiently manage any escalations that arise.
  4. Support members throughout the oncology care continuum, from screening to survivorship or end-of-life care, coordinating care and providing clinical support as needed.
  5. Identify and promptly address member needs, providing assistance with care coordination, symptom management, nutritional support, discharge planning, and provider referrals.

Skills

Required

  • Active, unrestricted compact Registered Nurse (RN) license
  • Degree in Nursing (ADN or BSN)
  • At least 5 years of nursing experience
  • 3 years in solid tumor oncology nursing
  • Oncology-related certifications (OCN, AOCN, AOCNS, or CCM) required or obtained within 2 years of hire
  • Strong communication and interpersonal skills
  • Organizational skills and ability to multitask
  • Adaptability and comfort with dynamic environments

Nice to have

  • BSN preferred
  • Compact and single-state licensure in New York and/or Illinois preferred
