Oncology Transitions of Care Nurse (11:30am - 8:00pm Est)

Thyme Care is seeking an Oncology Nurse - Transitions of Care to join their team. This role will be on the frontlines serving members diagnosed with cancer, conducting clinical assessments, monitoring health changes, coordinating care transitions, and educating members and caregivers over the phone. The role emphasizes a member-first approach, strong clinical skills in oncology and case management, and comfort with technology.

What you'd actually do

  1. Conduct clinical assessments, monitor for changes in health, coordinate care, including transitions, and educate members and caregivers about their diagnosis and treatment over the phone to support our members as they move through the oncology care continuum.
  2. Communicate member and provider feedback to our clinical leadership.
  3. Assist with other administrative projects as needed.
  4. Coordinate discharge plans with hospital case managers and follow-up care with providers.
  5. Monitor member progress, provide regular updates, and establish targeted support plans with the healthcare team in case conferences.

Skills

Required

  • Active, unrestricted compact Registered Nurse (RN) license
  • Degree in Nursing (ADN or BSN)
  • At least 5 years of nursing experience
  • 3 years of oncology nursing or case management experience
  • Certified Case Manager (CCM)
  • Strong clinical focus
  • Culturally competent care
  • Effective listening and communication skills
  • Organization and ability to work under pressure
  • Comfort with ambiguity
  • Willingness to obtain additional state licenses as needed

Nice to have

  • BSN preferred
  • Experience with video chatting, Google Suite, Slack, electronic health records, or comfort in learning new technology
