Palliative Oncology Nurse Navigator

This role is for a Palliative Oncology Nurse Navigator at Thyme Care, a healthcare company focused on improving cancer care. The role involves providing high-touch human support to members with serious illness, combining it with technology and AI. The nurse navigator will conduct conversations about goals and values, advance care planning, and palliative care, assess symptoms, support members longitudinally, and facilitate end-of-life care discussions. The role requires an active RN license, experience in palliative care and oncology, and strong communication and organizational skills. While the company uses AI and technology, this specific role is focused on direct patient care and support, not AI development or research.

What you'd actually do

  1. Conducting conversations about goals and values, advance care planning, and palliative care via phone, email, and video communication.
  2. Conduct comprehensive symptom assessments, support members with serious illness longitudinally, and facilitate end-of-life care discussions, when appropriate.
  3. Actively engage with members, addressing clinical issues and efficiently managing any escalations that arise.
  4. Conducting a minimum of 20 member calls per day.
  5. Support members with serious illness and at the end of life, coordinating care and providing clinical support as needed.

Skills

Required

  • Active, unrestricted compact Registered Nurse (RN) license
  • New York, Illinois and/or Michigan licensure
  • Degree in Nursing (ADN or BSN)
  • 5 years of nursing experience
  • 3 years in outpatient palliative care and/or home hospice taking care of oncology patients and their caregivers
  • Fluency in Spanish, both written and verbal

Nice to have

  • BSN preferred
  • Certified Hospice and Palliative Nurse (CHPN)
  • oncology-related certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS)
  • Certified Case Manager (CCM)

What the JD emphasized

  • active, unrestricted compact Registered Nurse (RN) license is required
  • New York, Illinois and/or Michigan licensure preferred
  • at least 5 years of nursing experience, including 3 years in outpatient palliative care and/or home hospice taking care of oncology patients and their caregivers
